Assessment & Report Types

NDIS Case Reviews

As with other case reviews, a NDIS participant can require advice as to whether they are eligible for certain funding based on their functional presentation. This will advise clients of the assessments they require to obtain the funding results they need to reach their potential and maintain their choice and control.

Adult Functional Capacity Evaluation (FCE)

This report is comprehensive and provides recommendations within specific funding categories to meet the client’s needs and obtain their NDIS goals as outlined in their plan.

This assessment produces a comprehensive report that outlines the physical and emotional limitations of an adult’s disability on their activities of daily living.

Activities of Daily Living include:

  • Work

  • Leisure

  • Learning

  • Personal care

  • Socialisation

  • Community Engagement

  • Domestic tasks including yard maintenance

It utilises various assessment tools to determine the amount of care that a person may require to best meet their NDIS goals, and additional therapeutic support required.

The assessment also takes into consideration if the individual requires equipment to complete tasks, or modifications to home, or work environments.

This assessment considers the impact on carers, and the psychological impact of the injury on their life.

Interview and observation (approximately 2 hours)

Functional Capacity Evaluation (FCE) - Paediatric

Children have very specific functional needs and our senior clinicians has extensive experience in provision of paediatric and early intervention services within the NDIS framework. Her behaviour practitioner experience also supports the team to clearly document how behaviours of concern are impacting on functionality.

This assessment produces a comprehensive report that outlines the physical and emotional limitations of a child’s or young person's disability on their activities of daily living.

  • Playing

  • Learning

  • Personal care

  • Socialisation

  • Community engagement

  • Self help tasks

It utilises various assessment tools to determine the amount of care that a child may require to best meet their NDIS goals, and additional therapeutic support required.

Supported Independent Living (SIL) or Independent Living Options (ILO)

Living (known as SIL) is a NDIS funded support that provides help and/or supervision of daily tasks to help you live as independently as possible, while building your skills.

SIL happens in your home – usually in a home you share with other NDIS participants.

Individual Living Options (ILO) are living arrangements that focus on the individual. They include Living Alone, Co-Residency, Host Arrangements and Living Together options. They are an alternative to group home services.
ILO design features include individual focussed supports.

JRTS focuses this assessment and report on exactly the level of care a person requires to live in either of these settings, and makes sure that all functional and behaviour needs are taken into consideration.

The assessment tools used include:

  • Interview and observation (approximately 2 hours)

  • Sensory Profile Measure (SPM)

  • Care and Needs Scale (CANS)

  • Carer Strain Index (CSI)

  • Independent Life Skills Profile (ILSP 16)

  • Adaptive Behaviour Assessment Scale (ABAS)

  • SIL or ILO care and support table

The report is very comprehensive, and we work closely with clients, families and support coordinators plus other clinicians involved to include all aspects of the persons support model.

Assistive Technology (AT)

The funding in your plan is for you to purchase Assistive Technology devices which are considered. There are 3 different levels of Assistive Technology, and JRTS clinicians can support  with obtaining funding for all of them: 

Basic (Level 1) low-risk, off-the-shelf items < $1000 in value

This Assistive Technology are low cost/low risk products from local retail suppliers and need a letter of support from an occupational therapist. (e.g., shower tools, wheelie walkers, eating utensils and iPads to assist with communication).

Standard (Level 2) medium-risk, standard design and sizes < $15000 in value

This Assistive Technology involves specifically selected items that best suit the individual’s needs. (e.g., transit wheelchairs, mobility scooters, lounge chairs and hospital beds). This process requires support from an occupational therapist and an AT Advice Summary report.

High Risk (Level 3), customised items > $15000 in value

This Assistive Technology is specifically scripted by an occupational therapist, in conjunction with a product specialist. 

This process requires an Assistive Technology report to be completed by an occupational therapist, along with quotes to justify the purchase (e.g., power wheelchairs and specialist seating equipment).

JRTS clinicians are very methodical in their approach to equipment prescription, and ensure the process is seamless, and actioned promptly to avoid stress for all parties.

Once the report is submitted to NDIS, the client and/or support coordinator is notified when funding is approved. JRTS is happy to be involved in the delivery process to ensure everything meets requirements.

Specialist Disability Accommodation (SDA)

 Specialist disability accommodation (SDA) is a range of housing designed for people with extreme functional impairment or very high support needs. SDA dwellings have accessible features to help residents live more independently and allow other supports to be delivered with optimal safety.

JRTS has an excellent ability to compile SDA reports so that every funding option is explored. The assessment takes 2-3 hours to complete, either in person or online.

Our clinicians are very familiar with SDA legislation, and understand the complexities of this report structure. Although SDA is somewhat challenging to get approved, JRTS has an excellent track record thus far.

The report includes the following assessment tools:

The report is the most comprehensive of all our reports, and we work closely with clients, families and support coordinators plus other clinicians involved to include all aspects of the individual’s support model.

This ensures the correct SDA is obtained, and an individual's goals can be met in a person-centred and holistic way.

NDIS Access Reports

Obtaining access to the NDIS can often be a bewildering process! JRTS clinicians can advise and support individuals and their families to ensure all information is obtained, and presented in an appropriate manner to support access. 

Our clinicians are familiar with the legislation, and understand the correct assessments to use to outline the impact of disability on functionality. This process involves an interview for approximately 1.5 hours and the production of an access report that can get a NDIS application over the line!
